ADMINISTRATIVE REQUIREMENTS FOR INDIVIDUAL

  1. Application letter addressed to the Director General of CHUB

  2. Copy of passport for foreigners / Id for Local students

  3. Recommendation letter signed by dean/ Leadership of University /Institution

  4. Valid vaccination Certificate for Hepatitis B

  5. Personal health insurance coverage during study period

  6. Clear goals for experiences and Objectives of placement (why and what do you want to study here) (APPLICATION FORM)
